Privacy Policy
NOURISHED PROPERLY collects personal information, including personal health information, in order to administer its services. We are committed to protecting the privacy of our clients. All of our agents (practitioners, volunteers, etc.) are required to maintain confidentiality and handle your information in accordance with this policy.

This policy applies to all of the personal health information that in the possession or control of NOURISHED PROPERLY. Personal health information is identifying information about a person relating to their health or health care, including but not limited to their health status, medical history, health care provision, health care provider(s), or payments for health care.
All of the personal health information we collect is used to provide services. We do not sell or disclose your information to any 3rd party. We will only disclose your personal health information to a specified party with your written consent or where required by law. Personal information (e.g. name, email, phone number, address), other than personal health information covered by this policy, is also kept confidential and only used for the purposes for which it is collected.

NOURISHED PROPERLY maintains personal health information in a combination of paper and electronic files. Paper records are kept at our business address and electronic records are encrypted. Please be advised that email messages are a less secure means of communication. We only communicate your personal health information with you via email if you have consented.

You are entitled to request access to your personal health information. You will be notified of a decision within 30 days unless an extension is appropriate (up to 30 additional days). There may be a fee associated with accessing your information, in which case you will be provided with a quote.

We do our best to maintain the accuracy of your records; however, we rely on you to disclose all relevant changes to your information. If you find an error in your health information, please notify us to make the appropriate correction. We are not required to change information consisting of professional opinions or observations made in good faith.

You have the right to withhold or withdraw your consent for the collection, use or disclosure of your personal health information for health care purposes at any time.
